Hirschfield, BA, CCRC, co-founder of LMWP states, "we are a compassionate group of leaders, volunteers, and philanthropists who are committed to improving population health and welfare everywhere”.
Our community-wide plan embraces unmet healthcare needs. Lifestyle providers support wellness education in schools, workplaces, and healthcare institutions. We understand that a sick healthcare system worsens health inequities and we are committed to reversing this trend. Lifestyle Medicine Wellness Partners mission is to support "community wellness hubs" and improve health disparities for all people.
